According to Foot Mercato, the deal has been finalised and the winger is expected to travel to northern France in the coming days to complete the move.
Hazard became a free agent this summer after his contract with Anderlecht expired at the end of the 2025/26 season.
The 33-year-old is set to become Lens' second signing of the window following the arrival of Michael Cuisance from Hertha Berlin, who was recruited after captain Adrien Thomasson's departure.
Despite entering the latter stages of his career, Hazard enjoyed an outstanding final campaign at Anderlecht, registering 15 goals and 11 assists in 46 appearances.
