The Spurs academy graduate is yet to make a first-team appearance for the club but was named among the substitutes for their 2-0 win over Dortmund last November.
The 20-year-old was then shipped out on loan to Bristol Rovers, where he made 19 appearances, scoring six goals and providing one assist.
Akhamrich has now joined Leyton Orient on loan for 2026-27, becoming their sixth signing of the summer.
"I had a really positive loan last season and now I want to test myself at League One level," Akhamrich told the club website.
"I like to excite people when I get the ball. I'm a very positive and direct player and I hope that I can help us to have a good season."
