The Watford-born one-time Scotland international fired home in the 80th minute as fans of the Posh rejoiced at Old Trafford with immediate promotion following last season's relegation to League One.
Tommy Rowe and Grant McCann scored either side of Mackail-Smith's 35th goal in all competitions as Peterborough joined Brighton & Hove Albion, Southampton, West Ham United, Blackpool and Birmingham City as the new clubs in England's second tier.
