Hazard was taken off injured during Real Madrid's 1-0 loss against Levante on Saturday evening.
The Belgian only returned from a three-month absence with an ankle injury against Celta Vigo last weekend, but he didn't last much longer than an hour at the Estadi Ciutat de Valencia.
After limping around the pitch for a few minutes, he was substituted by Zinedine Zidane.
Zidane had a word with the forward, who was down on his haunches, on the touchline and there were worried faces all round.
Once he'd taken his place on the bench, the former Chelsea player was given an icepack to put on his right ankle - the same one that had kept him out for three months.
