The Bayer Leverkusen star has reportedly informed the club about his desire to move in the coming transfer window, according to German reports.
The Evening Standard says Frank Lampard has identified Havertz as a key target this summer.
He sees the creative youngster as the final piece in the jigsaw of his team's attack for next season and beyond.
Havertz is a hot property, and is wanted by the likes of Bayern Munich and Manchester United as well.
The Blues may have to pay £70 million or more to seal the transfer.
