Abraham came off after 60 minutes against Bolton - three minutes after becoming the first player since Andy Gray in 1977 to score 25 league goals in a season for the Villains.
After the 2-0 win, Villa boss Dean Smith said: "Tammy got a whack on the shoulder. It's too early to diagnose but I don't expect he will be fit for Monday. I hope it's not bad.
“He put his head in where somebody with 25 goals puts their head in.
"He was down for a while. The doctor said he had to come off because he was in too much pain."
Abraham is due back at parent club Chelsea for pre-season.
