Il Mattino says Real have approached Napoli and asked if they wanted to take Illarramendi on loan for this season.
Azzurri coach Rafa Benitez wants to wait until after the second leg of the Champions League play-off with Athletic Bilbao on Wednesday.
Illarramendi has also attracted an approach from Athletic in the past week.
