The Daily Star says Spurs staff expected the 32-year-old to recover quicker but he has been limited to work in the gym.
And boss Andre Villas-Boas fears the ex-West Ham star could be out until the festive period.
That would also rule him out of England’s friendly in Sweden next month for his first Three Lions appearance since going to the Euros as a first-choice player.
“It’s difficult for him as he still feels pain and is still disturbed in his recovery,” said AVB.
“It prevents him from being on the pitch but he has been in the gym with the medical department. We could be looking at Christmas. Hopefully less.
“It is still within the limits of what we expected. He is making good progress.”
