Manchester City have broken FA anti-doping regulations three times in less than five months.
The club also failed to heed a written warning sent three weeks before their third and final breach of the rules.
City were fined £35,000 last month for breaking the FA's 'whereabouts' rules but the written reasons for the verdict have now been published.
Under anti-doping rules, all clubs must provide the FA with accurate information about training sessions and players' addresses so drug testers can find them.
