The 22-year old full-back spent 93 days with the Terriers, making nine appearances, but is now back in south Wales after receiving praise from Town boss Mark Robins.
"We'd all like to thank Jazz for his contribution during his time at the club," Robins told the club's official website.
"He's a good footballer and did well for us when called upon. Even when he found himself out of the side, mostly due to our change in formation and the form of others around him, he was still a part of the first-team squad and was a consummate professional."
