Carlos Bacca is poised to join Italian giants AC Milan.

The Colombian striker will leave Spanish side Sevilla to sign a contract with the Rossoneri once he completes a medical in his home country under supervision from Milan officials.

Bacca, 28, has been heavily linked with Premier League pair Liverpool and Manchester United but it is widely reported that Milan have met his buyout clause of £21million.

Bacca has netted 49 goals in 108 appearances for Sevilla since arriving from Belgian side Club Brugge in the summer of 2013.
