Liverpool defender Kolo Toure took responsibility for their draw with West Bromwich Albion yesterday.
Toure admitted his mistake cost Liverpool victory at the Hawthorns, labelling his horror pass to goalscorer Victor Anichebe as "crazy".
Toure admitted: "I made a bad pass. I made a bad decision. I don't know why I have passed it into the middle of the pitch, it was crazy.
"I haven't seen him. I should have just played it wide to Aly (Cissokho) but you make decisions sometimes and it's difficult because my pass just put the team in a difficult situation.
"I'm disappointed. The manager knows I'm disappointed. We have missed an opportunity. We just didn't need that kind of mistake."
Manager Brendan Rodgers said: "Kolo is devastated. It's unfortunate. We made a mistake and were punished. It was a good finish by Anichebe but it's cost us two points."
