He will go under the knife in Barcelona with specialist Professor Ramon Cugat over the next few days.
"I will face this challenge the way I always took on my injuries. I struggled to accept it at first, because I was going through such a good period of form, especially after the brace against Real Madrid," he told Milan Channel.
"At the age of 37 it is a tough blow, but I want to return and I will celebrate again. I thank everyone for their affection and I wanted to personally read all the messages of support from the fans.
"I told my teammates that I want to come back for the final game and celebrate something important. Their support really moved me.
"I will really miss the atmosphere of the squad, as I see such a positive attitude. My teammates are strong and we are so eager to conquer an important trophy."
